NativeVision Year Round Program Registration

September 2018

Dear Parent/Guardian,

We are excited to offer the NativeVision Year Round Program at your child’s school. NativeVision is a fun and enriching fitness, nutrition and life skills program. The program occurs during school periods that have been approved by the teachers and principal. At some sites we also offer after-school programming.

In order for your child to participate fully in the NativeVision lessons and physical fitness sessions, we must have a completed Registration and Medical/Media Release Form. This website will direct you to this form.

We also have a research portion of the program to monitor the impact of NativeVision on participating youth. Please review the attached Parent Permission Document which requests your permission for your child to complete a survey. At some sites we also have an Informed Consent Document for parents, which requests your consent to complete a parent survey and parent visits to extend the NativeVision impact to the whole family. This website will direct you to these forms.

We aim for all participating students to be present for the full school year. We will be working towards goals and providing incentives for participation. We will also be sending information home to share what your child is learning and working on. We look forward to parent involvement in the program.
